Oral estrogen free contraception

ABSTRACT
Background: Progestogen-only pills have traditionally been associated with insufficient
cycle control, strict rules for intake and with exclusive indication for women
with estrogen contraindications.
Objective: To review the evolution of progestogen-only contraceptives in relation to
their formulation, limitations and indications.
Method: Search PubMed bases for original articles published between 1980 and
2019 with the MeSH: Estrogen-free oral hormonal contraception; Progestogen-only
contraceptives; estrogen antimineralocorticoid; Drospirenone
Results: 65 articles were found and 34 were discarded due to limitations in the
methodology, limited or non-representative study populations.
Conclusions: Progestogen-only contraceptives, which contain levonorgestrel, norethisterone
acetate or desogestrel, have traditionally been associated with strict schedule
rules for intake and a suboptimal bleeding pattern. Drospirenone is a progestogen and
estrogen-free contraceptive at a dose of 4 mg in a 24/4 regimen. The molecule has
antigonadotropic, antimineralocorticoid, antiestrogenic and antiandrogenic properties.
Clinical studies have demonstrated efficacy comparable to combined oral contraceptives,
a zero rate of cardiovascular side effects, in particular thromboembolic events
and a favorable bleeding pattern.
