Airway in the polytraumatized patient: use of videolaryngoscopy as an alternative and solution

ABSTRACT
Severe polytrauma is a public health problem with an overall mortality up to 12%, is the leading cause of death in individuals 18-44 years of age worldwide. In Mexico, the National Institute of Statistics and Geography, accident reports fourth cause of death. Improving trauma patient survival it is associated with early endotracheal intubation during prehospital care. About 9.9% of trauma patients require early intubation in the first two hours of arrival at the trauma center. Tracheal intubation remains the gold standard for the management of the airway in trauma patients, and should be made orally to support a rapid sequence intubation and cervical manual stabilization maneuvers. Trauma patients have their own causes of difficult intubation. The incidence of difficult airway in them is 7-10%, with an intubation failure of 11.6%. Assisted video laryngoscopy combines the advantages of conventional laryngoscopy and bronchoscopy. It allows vision of the glottis in a visual field of 45-60 degrees. It may be useful in patients with cervical spine.
