Our initial experience in laparoscopic gastric surgery

ABSTRACT
Introduction: Gastric cancer is the second most common cancer in the world; it shows male predominance in the seventh and eighth decades. Its incidence in Uruguay is 6.4 to 14.26/100,000 with a mortality of 4.79 to 11.37/100,000. The prognosis is directly related to the stage at diagnosis, and survival depends on the degree of invasion depth and nodal involvement. The laparoscopic approach of locally advanced gastric cancer is proving to be an excellent tool for the treatment of this neoplasm. It gives oncological safety, allowing for equal or more extensive resections that laparotomy surgery. Technically, extensive training and learning curve are required to make it possible.
Material and methods: We analyzed the medical records of patients who underwent laparoscopic gastric surgery in MUCAM and the Surgical Clinic “B”. 21 patients were operated on between July 2009 and July 2013. We discuss technical aspects of laparoscopic gastrectomy and analyze our results.
Results: We operate on 13 men and eight women, with a median age of 64.5 years. We made 9 total gastrectomies, 11 D2 lymphadenectomies, being 12 of them curative surgeries. Two thirds had locally advanced tumors. An average of 25.5 nodes were resected, and 29.8 in D2. 60% of positive nodes at the time of surgery.
Conclusion: The management of gastric cancer requires a multidisciplinary team, and all patients should be discussed in a Cancer Committee. Oncological radicality is demonstrated, all tumors were resected with negative margins (R0) and lymphadenectomy was satisfactory, with morbidity comparable to international series.
