Characterization of patients with pneumonia associated to artificial mechanical ventilation

ABSTRACT
Introduction: Intensive Care Units (ICU) improve survival because of treatment offered by professionals with higher qualification, but with the risk of adverse effects like ventilator associated pneumonia.
Objective: To characterize epidemiological and clinical variables, to determine the relationship between risk factors vs. status at discharge and the causative organisms.
Method: We performed a descriptive, prospective, transversal study, between September 1, 2009 and September 30, 2010, in the intensive care unit of the General Docent Hospital: "Dr. Enrique Cabrera" in 58 patients with ventilator-associated pneumonia.
Results: The patients over 60 years (62.1%) and males (70.7%) predominated. Chronic obstructive pulmonary disease (COPD) was the more frequent pathological personal finding and the diagnosis at admission (51.7%). 63.80% of patients were ventilated more than seven days and 74.14% had a length of stay over seven days. Reinfections were reported in 28 patients (48.28%) along with 98 episodes of ventilator-associated pneumonia. The risk factors found were prior antibiotic use, age over 60 years and hypoalbuminemia in 87.93%, 62.04% and 60.34% respectively.
Klebsiella pneumonia (30%) was the more frequent isolated causative agent when clinical suspicion existed and
Acinetobacter baumannii led the new episodes of reinfection.
Conclusions: Ventilator associated pneumonia continues to be a major problem in the ICU.
