Metformin and its current applications in the clinical setting

ABSTRACT
Metformin began to be used in the treatment of diabetes mellitus type
2 in Europe in 1957 and in the United States in 1995. Recently this
drug has been the mostly prescribed oral anti-hyperglycaemic agent
all over the word. In 1998 the United Kingdom Prospective Diabetes
study demonstrated the anti-atherogenic effects of metformin and later
it was discovered that the components of the syndrome of resistance
to insulin (metabolic syndrome) were reduced with metformin. The
Diabetic Prevention Program demonstrated the potential of metformin
in the prevention of diabetes. Its efficiency, safety, multiple cardiovascular
and metabolic benefits and its capacity to be combined with other
antidiabetic drugs, which included insulin, have converted metformin
the first option for the treatment of patients with diabetes mellitus type
2. In the last years it has been evident the role of metformin in patients
with VIH and diabetes, fatty liver, as well as non diabetic patients mainly
those with metabolic syndrome, with impaired glucose tolerance and
women with desire to get pregnant, but suffer from polycystic ovarian
syndrome. The role for prevention and treatment of cancer could be
another development line of metformin in the future. The objective of
this review is to update the new options for the use of this drug.
