Tratamiento de la litiasis renoureteral con litotricia extracorpórea con ondas de choque en niños

ABSTRACT
Introduction: infantile urinary lithiasis is an uncommon disease if it is compared with the adult incidence.
Objectives: to describe the result of treatment applied in Cuba in children presenting urinary tract lithiasis be means of shock waves extracorporeal lithotripsy (ESWL).
Methods: sample includes 202 children seen in the Center for Treatment of Urinary Lithiasis of the «Hermanos Ameijeiras» Clinical Surgical Hospital of Ciudad de La Habana from April 1986 to December 2009, using three types of lithotripters: the HM3 (Dornier), LIYHOSTAR PLUS (Siemens) and MODULITH SMX-MX (Storz).
Results: limit ages were 2 and 16 years. There was predominance of male sex 68 %. The predominant symptom was the nephritic colic. The more frequent background was the urinary infection, renal calculi 85.15 % and at the ureter 14.45 %. The mean surface was of 1.20 cm
2. Orotracheal general anesthesia was used 38.61 % and the intravenous one 61.39 %. In 7 cases; there were necessary re-treatments 3.47 %. Complications included acute urinary infection 2.97% and the renal subcapsular hematoma 0.99%. The 96.53% was free of lithiasic material.
Conclusions: the therapy by extracorporeal shock-wave lithotripsy (ESWL) was effective and with a low rate of mortality and of severe complications, thus, it is necessary that this technique be proposed as the first therapeutical option in the resolution of urinary lithiasis at these ages. We must to remember that is not enough to eliminate the calculus, we must to identify the disease, its base treatment and to avoid relapses.
