Antibodies seroprevalence against Coccidioides spp. in patients with clinical diagnosis of tuberculosis from Ensenada, Baja California, Mexico

ABSTRACT
The current epidemiological situation of Coccidioidomycosis (CM) is not known in detail in northern Mexico and its report is not required. Baja California is an endemic area with high prevalence of tuberculosis (TB) and HIV/AIDS, disease that could be indistinguishable from those of CM. Serological testing has been used as a diagnostic test in case of clinical suspicion of active disease. The aim of this study was to explore the prevalence of IgM and IgG anti Coccidioides in patients with clinical diagnosis of TB in Ensenada, Baja California.
Material and methods. We performed a serological diagnosis in 38 clinical samples from patients previously diagnosed with pulmonary or bone TB, with failure or relapse with antituberculous treatment in two referral hospitals in Ensenada, Baja California.
Results. 39% of the samples were positive for antibodies against Coccidioides spp. surface antigens, of these, 87% were positive for IgM, 4% were positive for IgG. and 4% were positive to both antibodies. No positive samples were detected HIV-1 and 2.
Discussion. Approximately 40% of the analyzed patients previously diagnosed with TB had antibodies against Coccidioides spp., suggesting a likely increase in the rates of CM in Baja California. Only one patient was positive to both antibodies, but with a negative culture and baciloscopy of
Mycobacterium tuberculosis. This would be a sign of disseminated primary CM or reactivation of subclinical under diagnosed previous disease.
