Acquired-Community Pneumonia. Validation of a prognostic instrument in a mexican cohort

ABSTRACT
Background: The use of the Pneumonia Severity Index (PSI) has been used as an objective instrument to stratify the risk class of the Community-Acquired Pneumonia (CAP), and in this way determinate if a patient should be hospitalized or treat as an outpatient. In this study we intended to use it in our patients and validate it´s application.
Material and methods: This is a prospective, longitudinal, analytic and observational study, with all the patients with the diagnostic of CAP admitted to our hospital. The PSI was used to evaluate the severity of pneumonia. We followed the progress until the hospital dischart.
Results: We cases of CAP were reported. The medium hospital days were 7.76 ± 5.3. Three percent of the patients had severity grade I, 13% grade II, 43% grade III, 43% grade IV and 33% grade V. Seven deaths were reported, 100% of them were on severity grade IV and V.
Conclusions: The patients with severity grade I,II and III, could be dischart and treat has outpatients with relative security, on the other hand, patients with severity grade IV y V should be admitted to medical treatment for the risk of mortality.
