Afección cutánea por linfoma linfoblástico de precursores B. Estudio clinicopatológico e inmunohistoquímico de cinco casos

ABSTRACT
Background: Precursor B-cell lymphoblastic lymphoma (B-LBL) accounts for less than 10% of lymphoblastic lymphomas. Involvement of the skin of a B-LBL is relatively uncommon.
Objective: To describe the clinicopathologic and immunophenotypic study of fi ve cases of B-LBL with cutaneous involvement, only one of which showed bone marrow involvement.
Patients and method: Patients presented clinically with tumors located on the scalp and face, two of them with large ulcers. The medium age was 12.8 years (range 4-24); there were two males and three females.
Results: Histological examination showed in all cases that the dermis was diffusely infi ltrated (with no epidermotropism) with neoplastic small to medium-sized lymphoid cells, with round or convoluted nuclei, “blastic” chromatin and high mitotic rate All cases were positive for TdT and CD79a. Four of the fi ve cases were positive for CD10. CD 20 was weakly positive in four of the fi ve cases and four of fi ve cases showed strong positive nuclear staining for Pax-5. Ki-67 was 70-90% positive in every case. CD34, CD1a, CD3, CD5 and CD7 were negative in every case.
Conclusions: Immunophenotyping is essential to establish the correct diagnosis of precursor B-cell lymphoblastic lymphoma. Aggressive chemotherapy is associated with a potential cure.
