Comparative study of two techniques of localization under ultrasound guidance for infraclavicular approach

ABSTRACT
Background: Infraclavicular brachial plexus block (IBPB) is considered an attractive technique; however, it is used little. It is associated with complications such as vascular puncture and pneumothorax. Ultrasound (USG) provides real-time images, displays the path of the needle and the proper distribution of local anesthetic, increasing the safety of the approach.
Method: Prospective, cross-sectional, comparative, experimental, open, randomized, study of two groups. Group I: USG infraclavicular block, and Group II USG plus neurostimulation. Both groups received 0.75% ropivacaine at 3 mg/kg dose. The effectiveness was evaluated by the scale of the four «P’s».
Results: There was no difference between groups for the demographic variables and type of surgery. A significant difference was observed for latent period (Group I: 18.3/Group II: 22.8 min). USG alone showed a trend to decreased time in both motor (17.6/18.6 min) and sensitive (8.9/10.5 min) blocks. The block quality satisfactory in 93.3 per cent of cases in both groups, respectively (p = 0.69). No complications in either group.
Conclusions: Techniques of localization under ultrasound guidance (USG) is safe and effective, it addresses complications, and decreases the latency period; however, a similar success rate was observed in both techniques.
