Outcome in the elderly after being discharged from the Intensive Care Unit

ABSTRACT
Introduction: In the last two decades, adults older than 65 years have been admitted to the ICU every time more frequently. Is still being controversial, the quality of life and functionality after been discharged.
Objective: To know the quality of life in adults older than 65 years after discharge of the ICU.
Design: Prospective, longitudinal, observational study.
Methods: We considered all patients over 65 years old who ingresses to the Unit Care Intensive from de ABC Medical Center in the period from 2006 to 2007. Functionality status was defined when admitted to the ICU and 90 days after leaving the hospital through Katz and Lawton functionality scale. Proportions were obtained through Chi square test. Statistical significance was considered if p ‹ 0.05.
Results: It was analyzed 80 patients with mean age of 73 ± 8 (65–95) years, 55% male. 16% needed to be hospitalized in a second occasion. All the patients received early rehabilitation, and 34% continue with rehabilitation 90 days after discharged. Ninety days mortality was 2%. In order to Katz scale, there were absolutely functionalist the 70% of the patient at ingress, and the 43% at egress (p ‹ 0.001). Accord Lawton scale, there were absolutely independents the 51.2% of the patients at ingress, and the 22.5% 90 days after discharge (p ‹ 0.000).
Conclusion: We have a low mortality in the patient ingresses in the ICU. However, it is not reflected in the functionally preservation after discharge of the patient, because they are a detriment in the auto care capacity in all the patients, especially in the absolutely independents at ingress to the basics and the instrumentals activity. It is possible that the more emphasis in the physic rehabilitation decreases this detriment. It is important to make a larger follow up study.
