Management of abdominal wall lumbar hernias. Follow-up of its results in the mid- and long terms

ABSTRACT
Objective: To present the results, in the short, mid, and long terms from our cases operated of lumbar hernia.
Site: Specialized Center for the Treatment of Hernias, León, Guanajuato, México.
Design: Series of cases, observational, and descriptive study.
Statistical analysis: Central-tendency measurements, percentages a review measurement for the qualitative variables.
Material and methods: This study included the patients operated of lumbar hernia from September 1998 to April 2008 without follow-up missing. Studied variables: gender, age, IMC, added pathology, surgical access, size of the defect, evolution up to 1, 6, and 12 months and annually. There were interviews in the consulting room in April 2008 in order to verify the evolutions.
Results: Thirteen patients were operated. One of them was excluded. From the remaining patients, 9 were women and 3 were men, ranging from 33 to 66 years old (mean: 55.5 years old), 8 of them were postoperated of nephrectomy, one after a bone grafting, two operations were post-traumatic, and another one was spontaneous. There was a hernia defect measuring between 15.7 to 314 cm2. Every hernia was restored by thee Rives’ technique with a polypropylene net (or mesh) for hernias. Direct open access in 9 cases, open anterior approach in two patients, and intraperitoneal access in one case. Follow-ups ranging from 12 months to 9 years and a half. Three relapses up to date (25%).
Conclusions: Urologic surgery gave origin to most of these hernias. The open access and restoration with the use of a net or mesh and a technique free of tension offers acceptable results according to the very complicated condition of this entity, with a 25% of relapse.
