Fasciola hepática: Investigación clínico-epidemiológica

ABSTRACT
Background: Human fasciolosis is rarely diagnosed in Mexico, despite the fact that animal infection is a major veterinary problem. It is adquired by eating watercress or drinking contaminated water with Fasciola hepatic metacercariae. Sheep, cattle and other herbivores are the definite host, and fresh-water snails are the intermediate host.
Methods: The medical records and laboratory studies of the affected family were reviewed, as well as the abdominal computed tomography (CT) of the patient plus the results of the laparoscopic examination and the histopathology of liver-biopsy specimens. Diagnostic serologic tests for
Fasciola and the search for fluke’s eggs in stools and bile aspirate, as well as the familial and epidemiologic studies.
Results: A 35 year-old man was hospitalized because eosinophilia, fever, abdominal pain and fluctuating hepatic lesions. CT examination showed solid and cystic lesions throughout the right hepatic lobe of the liver. Biopsy specimen disclosed
Fasciola hepatic hemorrhagic necrosis, a palisading granuloma and eosinophilic infiltration. The serologic tests were positive and the characteristic operculated ova were seen in the biliary aspirate. The patient was treated with triclabendazol

Conclusions: The invasive Fasciola syndrome is characterized by fever, abdominal pain and eosinophilia, the levels of hepatic enzymes and bilirubins remained normal. With availability of more effective serologic test, it is time to determine the prevalence of infection in the Mexican Population.
