Algorithm for the anesthetic behavior of the tributary myasthenics of surgical treatment

ABSTRACT
To propose an algorithm for the taking of decisions in the anesthetic behavior of the tributary myasthenic patient of surgical treatment.
Material and methods: 340 patients were evaluated. Of the general anesthesia we was evaluated the hypnotic, the narcotic, neuromuscular blockers and the used doses. The function neuromuscular was quantified with an accelerometer. The neuromuscular blockers doses were 25% of the habitual doses.
Results: The variables of function neuromuscular showed longering values. This values increasing between 0.8 and 2.8 more prolongated especially with cisatracurium to dose 25 µg.kg
-1. We only use maintenance dose with mivacurium because it were compartated as an intermedium action neuromuscular blocker. The half value of the height of the twitch with vecuronium, atracurium and cisatracurium were between 31.5 ± 0.6 and 89.5 ± 0.3%, but in the mivacurium group was › 95%. The stocking of the quotient T

1 with vecuronium, atracurium and cisatracurium were 45 and 62%. In the mivacurium group was ≥ 81.7 ± 1.1% for what the patients’ 95% was extubates in the theatre.
Conclusions: The algorithm propose is easy way the behavior to continue in the tributary myasthenic patient of surgical treatment.
