Treatment of upper gastrointestinal bleeding due to peptic acid disease: omeprazol vs ranitidine

ABSTRACT
Introduction: Upper gastrointestinal bleeding (UGIB) has an incidence of 1% and a mortality rate of 5-10% of hospital admission in USA and Europe.
Objective: To evaluate the efficacy of intravenous omeprazole
vs intravenous ranitidine in patients with upper gastrointestinal bleeding.
Design: Prospective study.
Setting: Department of Internal Medicine of a tertiary care hospital of Mexico City.
Patients: Thirty two patients aleatorized in two groups: I) 16 patients (six women, ten men, mean age 64.5 yrs) received continuous infusion of ranitidine (300 mg every 24 hours) and II) 16 patients (five women, eleven men, mean age 67.5 yrs) received a initial bolus of omeprazole (80 mg) followed by 40 mg every 12 hours, during 3-5 days.
Results: UGIB was controlled in twelve (75%) patients of group I and in fifteen (93%) of group II (p = 0.33). Blood of transfusion need was 1.2 units for group I and 16 for group II. At endoscopy the most frequent diagnosis founded were erosive gastritis 8
vs 4, duodenal ulcer 3
vs 7, gastric ulcer 4
vs 2, gastroesophageal reflux 1
vs 3 patients, for group I and II respectively.
Conclusion: In this study the efficacy of omeprazole was similar to ranitidine, however the population studied was small. It is necessary further investigations.
