Solitary bone plasmocytoma in thoracic wall: a unusual presentation and a time bomb

ABSTRACT
Background: Solitary plasmacytoma of the bone is a disease with low frequency,
especially with involve of the rib. It is characterized by one or two bone lesions without
disseminated disease and it has association with multiple myeloma. Because of this,
on suspicion of solitary bone plasmacytoma, this last disease has to be discarded, in
order to improve the patient’s prognosis.
Clinical case: A 53-year-old male patient, with a history of inactive smoking, who
went to the emergency department with a clinical picture of three months of evolution
consisting of pain with pleuritic characteristics in the left hemithorax, concomitant with
wet cough, feeling of dyspnea, asthenia, adynamia and subjective weight loss, in whom
solitary plasmacytoma of the bone was diagnosed and, despite receiving adequate initial
treatment, suffered multiple myeloma in a matter of months.
Conclusions: Despite of an adequate management, treatment fails have been described,
such as multiple myeloma progress, lesion recurrence or development of new
lesions without multiple myeloma.
