Effect of the opioids upon the heart

ABSTRACT
Opioid peptides and exogenous opioid are known to exert important cardiovascular effects. Activation of specific receptors result in a potent cardio protective effect to reduce infarct size in experimental animal and to reduce cell death in isolated cardiomiocytes. Nonselective opioid receptors antagonist such as naloxone and a selective delta 1-opioid receptors antagonist, have been shown to inhibit the cardioprotective effect of ischemic preconditioning, a phenomenon in which brief period of ischemia protect the heart against a more prolonged period of ischemia. Selective delta 1 agonist have been shown to exert potent cardioprotective effects in intact animals and cardiac myocytes via activation of G i/o proteins, protein kinase C, and mitochondrial Katp channels. Opioid have been used clinically to manage pain peri-and postoperatively. The demonstration that opioid receptors, most notably delta 1, which not only have analgesic properties, but may now have the potential to protect the myocardium during cardiac surgical interventions, suggests a possible new pharmacological approach for the treatment of patients suffering from acute or chronic myocardial ischemia.
