Neuroimmunological study in patients with spastic paraparesis

ABSTRACT
Introduction: Spastic paraparesis is a neurological disorder that according to its
etiology can be classified in different ways: progressive spinal paraparesis, hepatic
spinal paraparesis or tropical spastic paraparesis.
Objective: To characterize the neuroimmunological response, evaluated by
reibergram, in patients with spastic paraparesis.
Methods: A retrospective study was conducted in 2017, where the occurrence of
intrathecal synthesis of major immunoglobulins and the status of the blood / CSF
barrier in patients with spastic paraparesis was evaluated. We worked with an
intentional sample of 20 patients according to the inclusion and exclusion criteria,
from the LABCEL serrachokary and, retrospectively, two groups of patients were
formed: 7 pediatric patients and 13 adults. The levels of albumin, IgA, IgG and IgM
were quantified by immunochemical nephelometry with kinetic analysis. The
reibergram was used to evaluate the occurrence of intrathecal synthesis and the
status of the blood/CSF barrier.
Results: All patients presented intrathecal synthesis of at least one of the major
immunoglobulin classes. The most common synthesis patterns are those of two and
three classes. The immunoglobulin that was most synthesized was IgM. All pediatric
patients showed hundreds of high intrathecal IgM synthesis. The barrier dysfunction
only occurred in two cases, one pediatric and the other adult.
Conclusions: Inflammatory process of the central nervous system is noticed in all
patients.
