Endovascular approach in big basilar tip aneurysm

ABSTRACT
Cerebral aneurysms are an abnormal localized dilation in the form of protuberances or balloons outside an artery, secondary to a weakness in the internal elastic lamina. There are various devices for endovascular use in aneurysms, platinum Coils that fill the aneurysm sac, promote thrombosis and exclude the aneurysm of the arterial circulation, thus reduce the risk of rupture and rebleeding, up to flow diverters that alters hemodynamics of the vessel and reduce the risk of complications. We present the case of a 63-year-old male patient with no history of cardiovascular, neurological or other chronic degenerative diseases who had a 3-day history of sudden onset, which consists of holocranial headache, after which he falls from his own height without loss of alertness, and the course of the next 48 hours is added to nausea, projectile-type vomiting and progressive deterioration of alertness. A classic clinical case of subarachnoid hemorrhage. In the present manuscript we compare the result obtained in the endovascular management of basilar artery aneurysm with those reported in the current literature. A prospective analysis of endovascular management with Coils embolization and Pipeline flow diverter of aneurysm on the tip of basilar artery, comparing it with reports of similar cases and a recently published bibliography on the topic, as well as with the follow-up in its evolution during the following two years by means of control angiographies and appointments in outpatient clinic. We conclude, the use of Coils together with a flow divider represents a good treatment option due to the satisfactory results obtained when relating the evolution of our patient with what is described in those referred to in the bibliography consulted.
