Abdominal actinomycosis: differential diagnosis of ovarian malignant tumor

ABSTRACT
Actinomycosis is a bacterial infectious disease of infrequent presentation, produced by Actinomyces israelii. The symptoms are unspecific and its diagnosis difficult to establish. The majority are diagnosed with anatomopathological study, being «grains of sulfur» pathognomonic. The objective to describe a clinical case of abdominal actinomycosis, which simulated a neoformative process.
Clinical case: A 50-year-old woman, who comes to the clinic because of abdominal tumor, with intrauterine device. On physical examination, abdominal distension was documented, abdominal mass of approximately 10 centimeters, solid, not painful, bimanual exploration with uterus and annexes fixed. Computed tomography of the abdomen showed uterine myomatosis, increased dimensions of right annex, with adjacent inflammatory changes and mesenteric and retroperitoneal adenopathies of inflammatory type. Magnetic resonance reports pelvic tumor, without interphase with bladder dome and uterine fundus, suggestive of neoformative process of lineage to be determined. Uterine intramural and submucosal myomatosis. Adenopathies in internal iliac chains. Surgical intervention was performed, finding pelvic tumor dependent on uterus with infiltration to the small intestine and right colon, they were resected in block uterus and annexes, terminal ileum, right colon, bladder dome, implants in omentum and peritoneum. Pathology reported Actinomyces israelii of surgical pieces and intrauterine device. Treatment was started with antibiotics. Abdominal actinomycosis is a rare entity that should be taken into account in female users of intrauterine device and pelvic tumors, without symptomatology of infectious process.
