ABSTRACT
Hypothyroidism in pregnancy has a direct influence in maternal and neonatal complications. This guideline includes the most recent data about prompt diagnosis and correct treatment. An appropriate clinical approach is important because it can improve results in neurodevelopment and other short and long term neonatal outcomes. There is controversy about universal or selective screening among pregnant patients with hypothyroidism. Studies favor universal screening in the first prenatal visit. The diagnosis of hypothyroidism in pregnancy is made by measuring levels of thyroid stimulating hormone depending on varying values in each one of the trimesters. Thyroid hormones are of vital importance for neurodevelopment and cerebral growth. The most important maternal complications of hypothyroidism in pregnancy are pregnancy loss, preterm labor and placental abruption. Children with low levels of thyroid hormones have lower cognitive function. An adequate treatment can improve brainpower and long-term outcomes.
