Usefulness of simple chest tomography in patients with systemic lupus erythematosus

ABSTRACT
Background: The medical literature talks about complications and pulmonary, pleural and mediastinal comorbidity in patients with Systemic Lupus Erythematosus, however, there is no consensus that supports simple chest CT as a screening method for such disease.Objective: To identify the most common pulmonary, pleural and mediastinal findings and morbidities that may be found in the Mexican population having long standing SLE. To discuss the importance and usefulness of CT scan in these patients.
Material and method: A cross-sectional descriptive study was conducted at the Instituto Nacional de Cardiología Ignacio Chávez from April to June 2017, including patients diagnosed with Systemic Lupus Erythematosus of more than 10 years who had a simple chest CT to assess pulmonary, pleural, cardiovascular and abdominal findings, as well as in bones and soft tissue.
Results: Forty-one patients diagnosed with SLE (›4 ACR criteria) were included, 38 (93%) females and 3 (7%) males. Pulmonary findings were predominant, found in 82% of patients, 17% with nodules. In the cardiovascular system, 43.9% had systemic atherosclerotic disease, of these, 21% had a positive Ca Score, and 17% had pulmonary hypertension. In abdomen, splenic granulomas predominated.
Discussion: Chest CT is useful in patients with SLE for prevention of comorbidities, mainly cardiovascular and pulmonary, since they are more prevalent in the Mexican population.
