Balloon dilatation of the papila of Vater with sphincterotomy in choledocholithiasis difficult to extract

ABSTRACT
Choledocholithiasis is a complication of bile duct stones that occurs in 7-20% of patients with symptomatic lithiasis. Endoscopic cholangiography is the main method for treatment, with success rate over 90%, however there are factors that prevent its success. There is a technique for the treatment of «difficult bile duct stone» it consists in sphincterotomy plus dilatation of the papila of Vater in which success rates have been reported from 78 to 100%.
Objective: Investigate the clinical experience of success and complications of the extraction of difficult bile duct stones through sphincterotomy plus balloon dilatation in the Endoscopy Service of the Hospital Juárez de México in a period from May 2015 to May 2017.
Material and methods: Descriptive and retrospective study in patients undergoing to sphincterotomy plus balloon dilatation of papila as endoscopic therapy for the treatment of «difficult bile duct stone».
Results: 120 patients were included; the median age was 50 years. Successful extraction in 96.7%. The complications observed were pancreatitis and hemorrhage, both in 6.7%.
Conclusions: The success rate in patients with «difficult bile duct stone» is 97%.
