ABSTRACT
Rationale: The second edition of the Cuban ELAN Study of Hospital Malnutrition (completed between 2012 – 2014) revealed that 16.6% of the hospitalized patients fulfilled (at least) one indication for nutritional support. Higher rates of implementation of nutritional support schemes among those surveyed ones were observed. Objective: To examine the current state of the utilization of Artificial Nutrition (AN) techniques in Cuban hospitals. Study design: Retrospective, analytical. Material and method: The ELAN Study database was queried in order to recover data on NA schemes administered to hospitalized patients, access routes and nutrients solutions used, and duration time of the scheme. Results: Current rate of NA usage was higher: ELAN 2001: 2.2% vs. ELAN 2012: 10.9% ( = +7.8%; p < 0.05). NA schemes were distributed as follows: Enteral nutrition: 7.8% vs. Parenteral nutrition: 5.0%. Enteral nutrition schemes used a nasogastric tube for infusion of polymeric nutrients by gravity in 94.4% of the instances. Average duration of Enteral nutrition scheme was 9.0 ± 9.3 days. On the other hand, Parenteral nutrition schemes were administered through a central route in 70.0% of the cases. Dextrose solutions were used as the sole source of energy in 80.0% of the parenteral schemes. Current usage rate of parenteral lipids was lower: ELAN 2001: 40.9% vs. ELAN 2012: 18.3% ( = -22.6%; p = 0.05). In 4 of the parenteral nutrition schemes pumps were used for nutrients infusion. Average duration of the parenteral scheme 8.0 ± 9.2 days. Use of premixed nutrients bags was not observed in this survey, neither all-in-one admixtures. Conclusions: A higher AN usage rate was documented in the surveyed patients. However, prevalence of high-tech resources such as biocompatible feeding tubes, premixed bags and infusion pumps was low. Similarly, a reduced usage rate of lipid parenteral solutions was observed. Recommendations: Policies and actions should be adopted for the technological renewal of hospital AN schemes. In addition, continuous education programs aimed to the safe use of parenteral lipids solutions should be conducted in the hospital setting. Artificial Nutrition in Cuban hospitals. An update.
