ABSTRACT
The hepatoblastoma is the hepatic tumor more prevalent in infancy. The stage of the hepatoblastoma is based upon the
PRETEXT system. Stage IV is considered not surgical and confers the worst prognosis. The treatment of this stage is
based in neoadjuvant quimiotherapy and posterior partial or total hepatectomia plus hepatic transplantation,
accordingly with the POSTEXT.
We report the case of a toddler with stage IV quimiosensitive hepatoblastoma, with invasion of the retrohepatic vena
cava and pulmonary metastasis who underwent a right trisegmentectomy with resection of the retrohepatic vena cava.
This procedure was allowed by the presence of collateral circulation provide for the acigus vena, offering a curative
option for our patient.
The resection of the retrohepatic vena cava could be a feasible and useful alternative to allow partial hepatectomia with
clean boards, and offer a potential cure in selected patients
