Clinical implementation of the diagnostic unit infertility in “one-stop”: Full protocol, safe and well tolerated in the first consultation

ABSTRACT
Background: Traditionally, the study of infertile couple is a long and
slow process. The “One Stop” concept refers to perform the diagnosis
protocol in one visit.
Objetive: To evaluate the feasibility, safety and tolerability of a diagnostic
protocol performed on a first visit at the Infertility Diagnostics
Unit.
Material and Method: A prospective study. Couples underwent
medical history, women also was performed a transvaginal ultrasound,
hysteroscopy-sonohisterosalpingography and thyroid function study.
Patients were monitored to assess 30’ after for a vaso-vagal reactions
and the presence of pain was estimate using the numerical rating scale.
Male underwent semen analysis.
Results: The protocol was completed in 120 couples (98.36%). The
duration time was between 90-120 min. Thyroid stimulating hormone
(TSH) was ›4 _U/ml in 14 patients (12%). During the procedure or
30 min later, no pain referred 47 120 (39%) patients, 40 of 120 (33%)
mild pain, 25 of 120 (21%) moderate pain; pain 8 of 120 (7%) patients.
The mean pelvic pain was 5.2±2.8, 12 patients (10%) required
analgesia. Mild vaso-vagal reaction occurred in 7 cases (6%), one
patient (0.83%) presented severe vaso-vagal reaction. There were no
late complications.
Conclusions: This study shows that infertility diagnosis protocol
can be performed at the IDU in a one visit. The protocol is safe and
well tolerated, allowing the diagnosis, treatment options and knows
the reproductive prognosis of the couples. It is an attractive diagnostic
protocol for patients-doctors.
