Trabeculectomy in young adults with previous ocular surgery.

ABSTRACT
Objective: To know the long-term outcome of the intraocular pressure control after trabeculectomy in young adults with previous ocular surgery.
Methods: A retrospective study in young patients (age 15–45 years) with glaucoma was carried out. All patients had a previous history of intraocular or conjunctival surgery and underwent a trabeculectomy from January 1988 to January 1995. Success was defined as complete, if the postoperative intraocular pressure in the follow-up was 21 mmHg or lower without glaucoma medications and qualified as success if they required medications for its control. Failure was considered in those eyes that required additional antiglaucoma surgery, when ptisis bulbi developed or loose of light perception.
Results: 60 eyes of 54 patients fulfilled the inclusion criteria. Success rates were 46 (76.6%) at 1 year, 40 (66.6%) at 2 years, 34 (56.6%) at 3 years, 29 (48.3%) at 4 years and 23 (38.3%) at 5 years. Nineteen eyes (82.6%) were qualified as complete success and four eyes (17.4%) as success.
Conclusion: The success rate of the trabeculectomy in our study in young patients with antecedents of previous intraocular surgery is compared with the percentages reported in the literature regarding the same surgical procedure in young patients without previous ocular surgery. The high preoperative level of intraocular pressure and the presence of previous ocular surgeries were identified as significant predictors (or borderline) for the surgical failure. The trabeculectomy with mitomicine C in these eyes showed a discreet improvement for the intraocular pressure control from the end of the follow-up.
