Comparison of length of diabetes in patients with different degrees of retinopathy

ABSTRACT
Background: Although the odds of having diabetic retinopathy increase as diabetes duration does, this could be similar in different stages of diabetic retinopathy.
Objective: To identify the evolution time of diabetes by degree of diabetic retinopathy to determine it there are significant differences allowing for estimation of diabetic retinopathy stage, based on the former.
Patients and methods: Type 2 diabetics enrolled from 199 to 2001, with diabetes duration record and any degree of diabetic retinopathy were included. Patients were grouped according to diabetic retinopathy degree: group 1: mild nonproliferative diabetic retinopathy; 2, moderate nonproliferative; 3, severe or very severe nonproliferative, 4, proliferative diabetic retinopathy. Diabetes duration was compared between groups by one-way ANOVA.
Results: 389 patients were evaluated; 157 were assigned to group 1, 138 to group 2, 28 to group 3 and 66 to group 4. Diabetes duration mean was 13.1 years in group 1 (95%CI 11.8-14.3), 13.9 in group 2 (95%CI 12.7-15.2), 16 in group 3 (95%CI 13.5-15.5) and 16.02 in group 4 (95%CI 14.3-17.7); a significant difference was found only between groups 1 and 4 (p=0.0031). No difference was found in the remaining comparisons.
Conclusion: The results obtained do not allow for prediction of diabetic retinopathy stage from diabetes duration; this will be difficult till enough and precise data to determine diabetes duration are available. Early referral to the ophthalmologist is necessary, in order to have diabetic retinopathy graded.
