Surgical treatment in the broken intracranial aneurysms associated with cerebral infarction

ABSTRACT
Introduction: The association of an ischemic event in patients with an aneurysmatic subarachnoid hemorrhage, is frequent in neurosurgical practice and is found very related to the vasospasm, that appears in the initial stadia of the evolution of these cases; its approach implies a greater risk, as well as the need to apply some measures, to avoid transsurgical complications that can put in danger the life of the patient.
Material and method: An observational, descriptive study was carried out with patients operated in the Service of Neurosurgery of the General Hospital of Morón, Cuba, in the period between January 1996 and December 2003, which were bearers of broken intracranial aneurysms, in which stroke was associated, as aggravating factor.
Results: All the cases were handled in the late phase of the evolution. The aneurysms always were located in the anterior segment of the circle of Willis. The majority of the bags were stitched. Only a patient passed away, by acute heart attack of the myocardium after three months of postoperative follow-up with an excellent evolution.
Conclusions: The transsurgical neuromonitorization and some intervention medical measures help to obtain good results. The morbility is found related in greater measure with the clinical state at the moment of the surgery, than with surgical accidents.
