Treatment of coronal craniosynostosis with forward and three-dimensional craniofacial remodeling by osteogenic distraction

ABSTRACT
Our clinical experience has shown that osteogenic distraction in the jaw and other areas of the craniofacial structure is possible. The coronal craniosynostosis is characterized clinically by brachycephaly and decrease of the AP projection of the upper orbital rim, the inferior orbital rim and the medial and lateral orbital rims. We present our experience lengthening the cranial vault through gradual distraction using a mixed distractor in patients with coronal craniosynostosis. The surgical procedure consists of making a linear craniotomy in the site of the craniosynostosis, in this case, in the coronal synostosis. A self-retention distractor at the bitemporal level is placed and the process of craniofacial osteogenesis-elongation is carried out by activating the distractor 1 mm per day. The results reveal a tridimensional remodeling with an exorbitism reduction, decrease of the vertical diameter of the skull and increase in the AP diameter. Since the dissection of the dura mater is not performed, the brain has the ability to influence the tridimensional remodeling through the adhesion of the dura to the inner table of the skull. In addition, the method becomes less complex with a significant reduction in complications, without recurrence and less morbidity.
