Management of nasal tip by crown graft

ABSTRACT
Objective: To evaluate the efficacy of the crown graft in the management of nasal tip.Material and Method: A retrospective study including charts of rhinoplasty patients was conducted. Five independent observers analyzed pre and post-surgical photographs.
Results: Sixteen patients were included in the analysis, 11 women and 5 men. In 2 cases, the crown graft was placed during a revision rhinoplasty, and in the rest as part of a primary rhinoplasty. An improvement in nasal tip projection, rotation and definition was observed in all cases. Supratip break improvement was observed in 91% of the observations. Improvement in symmetry was observed in 57% and in columellar double break in 46% of the observations. None of the patients presented post-surgical asymmetry. In one patient rectification of columella was observed after the procedure. None of the patients had complications nor expressed post-surgical aesthetic dissatisfaction.
Conclusions: The crown graft has been used successfully in cases in which achieving nasal tip projection or definition can’t be done with suture techniques alone. It’s a graft that imitates the shape of the lateral crura and the aesthetic results are natural. It’s also useful in revision rhinoplasty in cases with tip asymmetry or external nasal valve collapse during inspiration.
