Abduction splint use and subsequent management in the Ponseti method

ABSTRACT
Congenital talipes equino varus (CTEV) is a complex deformity that is difficult to correct. Dr. Ponseti’s method is the most accepted treatment worldwide. Long-term studies show that it has a success rate of 90 to 95 percent. The Ponseti method protocol requires the use of foot abduction orthoses after the last cast has been removed. This step is very important for the success of the treatment because it strengthens the gastrosoleus muscle and stimulates the Achilles tendon. It is crucial to make the parents responsible for the adequate use of the Dennis Browne splints, thus maintaining the achieved correction of the foot. At this time, there are many types of abduction splints, varying in the materials used for their construction and price range, but all of them designed to maintain the foot in abduction and allow for dorsiflexion of the ankle. Different studies have shown that that there is a recurrence rate of up to 80 percent when the abduction splints are not used properly. Poor parent compliance and their educational level are two factors that influence this result.
