General anesthesia in a patient with morbid obesity

ABSTRACT
Morbid obesity is characterized by an anesthetic risk increase of morbidity and mortality. A 65 –year-old patient with morbid obesity associated with hypertension and diabetes mellitus that came to the Intensive Care Unit of the "Lenin" Hospital with acute cardiorespiratory failure and prolonged ventilation was presented in this paper. After two months, with septic eschar in the sacral region the patient was transferred to the Surgery unit where after daily treatment over a period of 30 days the patient was announced for closing and repairing the affected region. In the anesthetic preoperating evaluation he was compensated of his base diseases, with a corporal weight of 200 kilograms and when evaluating the airway he had a Mallanpati degree III with a short neck and scar of tracheostomy, and a physical state ASA IV according to the American Society of Anesthesia (ASA) classification. Conditions for a difficult intubation were created, two surgical tables were united, and he was premedicated and placed in left lateral position with supinal rotation of the trunk to favor the ventilatory mechanics and the access to the airway. The anesthetic procedures were made suitably. There were no perioperative complications.REFERENCES
