ABSTRACT
Primary neuroendocrine small cell carcinoma of the esophagus is a rare, aggressive and of poor prognosis tumor, combination therapy using chemotherapy, radiotherapy and surgery appear effective for them. The case of a male patient of 48 years admitted for difficult diagnosis weight loss and dysphagia to solids to aphagia, with evolution of 11 months, the endoscopic ultrasound refers irregular thickening of the walls of the esophagus to the adventitia with fistula formation and stenosis, submucosal appearance tumor not infiltrating adjacent organs and without lymphadenopathy, same result can be seen on CT, biopsies performed in 3 endoscopies are inconclusive, we decided to perform a transhiatal esophagectomy, carcinoma histopathology report refers small circumferential infiltrating esophageal submucosa and muscle layers, unaltered gastric fundus, 15 lymph nodes with lymphoid hyperplasia, tumor-free surgical limits cells. In IHC cells were positive for synaptophysin and chromogranin A labeling index and Ki-67 was 70 to 80%. These findings confirm the diagnosis of small cell neuroendocrine carcinoma, in subsequent management patient received adjuvant 5-FU and cisplatin, has been followed for 9 months without evidence of relapse.
