Prothrombin complex concentrate in the perioperative

ABSTRACT
Prothrombin complex concentrates are a purified drug products with hemostatic activity derived from a plasma pool. Prothrombin complex concentrates contain a given and proportional amount of four non-activated vitamin-K dependent coagulation factors, a variable amount of anticoagulant proteins and low dose heparin. Prothrombin complex concentrates offers a means for the rapid reversal of warfarin, particulary in the setting of life-threatening bleeding. Prothrombin complex concentrates are recommended as an alternative to fresh frozen plasma and recombinant factor VIIa to the treatment of serious bleeding related to vitamin K antagonist. Studies in this setting have show that prothrombin complex concentrates are safe and effective and provide prompt reduction of international normalized ratio. Other indications include coagulopathy due to hepatic failure, traumatic hemorrhage and new oral anticoagulants intoxication. Advantages of prothrombin complex concentrates include minimal disruption to haemostasis, rapid administration, fast onset of action and low adverse effect. The aim of this paper is to review current concepts related to prothrombin complex concentrates and their use in the perioperative period.
