Complicaciones de la histerectomía total abdominal ginecológica por patología benigna

ABSTRACT
Introduction: Hysterectomy is the most common gynecological procedure performed. Almost 90% of all hysterectomies are performed for benign indications, of which the most frequent leiomyomatosis. Like any surgery, abdominal hysterectomy is not without complications, especially if risk factors such as obesity, chronic degenerative diseases, previous surgeries, anemia and others are present. Infectious and hemorrhagic complications are the most frequent in most studies.
Objective: To identify the most frequent complications that occur in patients undergoing total abdominal hysterectomy for benign indications.
Material and methods: An observational, descriptive, cross-sectional and retrospective study. We included all patients who underwent total abdominal hysterectomy for benign gynecological indication in the Hospital of Gynecology and Obstetrics, Maternal-Child Institute of the State of Mexico from July 1, 2011 to June 30, 2013; those patients in whom the histopathological study reported malignancy were excluded.
Results: We included 438 patients, of whom 66 –15.05 %– had complications. The most frequent complication observed was hypovolemic shock, in 9.1 % of patients, followed by postsurgical infections, with 3.7% of cases, and wound dehiscence, in 3.2%.
Conclusion: Bleeding complications were the most frequent, followed by postsurgical infections; we found that the main risk factors associated with these complications were obesity, diabetes mellitus and hypertension, which shows the need to control these underlying pathologies prior to surgery in order to reduce the risk of complications.
