Candidemia behaviour in an intensive care unit

ABSTRACT
Introduction: Invasive candidiasis has increased and although
C. albicans is the most common, species as
C. non albicans antifungal resistant are emerging.
Objective: To characterize clinical and microbiologically the patients with candidemia.
Method: A longitudinal, descriptive and prospective study of 52 patients admitted between January/2006-December/2009 was conducted.
Results: 29 (55.8%) women and 23 men (44.2%). The mean age was 56.8±17.013 years and 26 had length of stay greater than 15 days, 34/65.4% had scores greater than 12 from Sevilla Scale. 35 patients died (67.3%), most (36/69, 2%) had C. non-albicans, 41 required mechanical ventilator assistance, 9 (22%) survived and 32 (78%) died (p = 0.003). There was an increased mortality in patients with C. non albicans (p = 0.016). Although antifungal resistance was not elevated, it was higher in the C. non albicans species. Multiorgan dysfunction was present in 43 (72.1%) who received specific treatment while 12 (27.9%) received empirical treatment. Among patients without multiorgan dysfunction, 7 (77.8%) were treated empirically and 2 (22.2%), with specific treatment (p=0.008).
Conclusions: The majority of the patients in the group with empirical treatment survived meanwhile the majority of the patients in the group who received the specific treatment.
