Anaesthesia for spinal surgery in adults

ABSTRACT
In the last decades, spinal surgery in Mexico has advanced in an impressive
way, with incoming of minimal invasion procedures and percutaneous
approaches, and, besides this, we are still performing major
procedures of multiple levels, for anatomical correction surgery or tumor
resection. Most of the times, neurophysiologic monitoring is routinely
used in these surgeries, so, the anesthesiologist must be familiarized
with the anesthetic technique of choice, the working drugs and the
position of the patient, which are conditions that the anesthesiologist
is able to modify, and may benefit the final result of the surgery. The
pre-anesthetic evaluation should be specially focused on respiratory,
cardiovascular, motor and sensitive systems, as well as to the airway,
which is frequently difficult to manage, due to different co-morbidities
in this kind of patients. Prone is the most frequently position used in
spine surgery. That´s why the physiological effects of this position, how
to diminish the incidence of respiratory complications, and even the
bleeding during the surgery will be described. The monitoring may be
different, according to the type of surgery or the chosen anesthesic
technique. Analgesia for spine surgery depends on the type of surgical
approach, and then analgesia is commonly multimodal.
