Early neonatal pneumonia, experience at Hospital Español de México

ABSTRACT
Early neonatal pneumonia occurs in the first 28 days of life and is still a risk of late complications and premature death. Objective: to review the clinical characteristics and outcome in neonates treated at a private hospital of Gynecobstetric and Pediatrics care.Patients and methods: We reviewed all new cases of early neonatal pneumonia diagnosed by clinical and radiological evidence in 2011 and 2012. Complications and mortality are also presented.
Results: 23 cases were treated (15 preterm and 7 term). Three patients died (13%) due to sepsis and disseminated intravascular coagulation. Thirteen patients (57%) were diagnosed within the first 24 hours of life. Thirteen had segmental pneumonia, the others with bilateral pneumonias. Four associated with atelectasis. Only two infants had an isolated germ in some culture (Blood cultures: Staphylococcus hominis-homin Staphylococcus epidermidis and one on bronchial secretion). Eleven had pneumothorax as a complication. Six were discharged with Bronchodysplasia (all preterm).
Conclusion: The incidence of early neonatal pneumonia was low. It was presented in pre-term or low birth weight infants. The current treatment has reduced its lethality, although still complications as prolonged ventilation and barotrauma observed during the treatment.
