Treatment of the metaphyseal distal fractures of radius in child by means of percutaneous fixation

ABSTRACT
Introduction. The metaphyseal distal fracture of the radius in the child is a common pathology in our society. Commonly this fractures has been treated by close manipulation and a brachipalmar cast. Although, frequently the fracture lose it’s reduction, fact that conduce to the orthopedic surgeon to perform a new procedure in order to avoid anatomical deformities and functional complications. Prevot and Proctor reported different criteria to perform fixation by percutaneous K wires.
Material and methods. In this retrospective research we report our experience performing reduction and percutaneous fixation in radial metaphyseal distal fractures in 52 children as a primary treatment. The inclusion criteria were: pediatrics patients with distal metaphyseal radium fracture. The exclusion criteria were: patients attended in other hospitals or patients in whom the fracture was already healed. The elimination criteria were: loosing of the patient or incomplete files. The surgical procedure was: With the patient under general anesthesia, we performed reductive maneuvers with fluoroscopic vision support. At once we reduced the fracture and fixated it with two 0.062 inches K wires in the articular facet of the radius. Finally we put a cast in a light flexion position of the wrist. Five or six weeks later, the patients were programmed to retire the wires and to mobilized the wrist under general anesthesia in order to improve the articular motion.
Results. We obtain good results. We report five minor complications that were solved. We consider that this treatment may have all the risks of a surgical procedure, offer very good results and the associate morbidity is low.
