Root hemisection. Interdisciplinary management. Report of a case

ABSTRACT
The purpose of this case is to report the interdisciplinary management of a patient with extensive decay in a lower molar, presenting root hemisection as a treatment alternative.
Description of the case: Male patient of 55 years of age, who attended the school of dentistry because of pain in tooth 46. The clinical examination exhibited an extensive decay on the distal part of this tooth. Radiographically, a radiolucent area involving furcation and part of the distal root was located. The root treatment was performed for the mesial root and the distal root was removed. Three weeks after surgery, the tooth received a provisional restoration and the patient was referred to the school of orthodontics to close the space previously occupied by the distal root. This case has 4 years of follow-up, the clinical and radiographic conditions are favorable.
Discussion: Several longitudinal studies suggest encouraging results on the long-term prognosis in this type of treatment, as long as the case is handled in an interdisciplinary way.
Conclusion: Root hemisection is an alternative to extraction and implant therapy. Professional maintenance, proper control of oclussal forces and correct oral hygiene by the patient are essential to the long-term success of this and any other dental treatment.
