Ultrasonographic diagnosis of the extracranial carotid stenosis

ABSTRACT
Introduction: knowledge on the degree of stenosis caused by atherosclerotic plaques in the extracranial carotid system allows adjusting the suitable therapeutic strategy to these sick persons. A steno-occlusive lesion in the carotid axes may be the source of embolism or the cause of hemodynamic reduction of the flow and of the blood pressure in the brain.
Objective: to update the information on stenosis in order to determine the ideal non-invasive diagnostic test for these lesions. Such a test should be able to identify the normal arteries, all degrees of stenosis including occlusion, and the superficial and structural characteristics of the plaque.
Methods: review articles and metanalysis from the last 10 years and indexed in PubMed database of the National Center for Biotechnology Information were analyzed. The main keywords were carotid stenosis, measurement and diagnostic.
Information synthesis: the validity of the non-invasive methods for the diagnosis of the carotid disease was confirmed on account of their diagnostic accuracy. The difficulty lies in comparing these results with those of the arteriography (method of reference) without bearing in mind that both are based in different principles; the former are based in physiological principles, and the latter on anatomical principles, therefore the stenosis percentages are not occasionally comparable.
Conclusions: the ultrasonography (mode B and Echodoppler) remains the non-invasive diagnostic option for carotid stenosis quantitation.
