Recurrent Respiratory Papillomatosis, Results of the Treatment with CO2 Laser in a Reference Center; Experience of Six Years

ABSTRACT
Background: Recurrent respiratory papillomatosis (RRP) has been
little studied in Mexico. Data on the incidence, course and treatment outcomes are scarce. We studied the population with this disease in a
referral hospital to determine the epidemiology and the results obtained
with CO
2 laser treatment and compared the results with those of the
international literature.
Objective: To determine the response to treatment and recurrencies of
respiratory papillomatosis in patients treated with direct laryngoscopy
and resection with laser CO
2.
Patients and method: A retrospective cohort study was done at Hospital
Central Sur de Alta Especialidad, Pemex, Mexico City, from January 1
st
2004 to May 31
st 2010. The cohort consisted of patients with a histopathologic
diagnosis of recurrent respiratory papillomatosis by direct
laryngoscopy and treated with CO
2 laser resection that will have a
minimum of six months follow-up and had a full file. We obtained demographic
and clinical condition pre, trans, and postoperative follow-up
for each patient. The study included 20 patients with recurrent respiratory
papillomatosis, 12 (60%) men and 8 (40%) women.
Results: Nine patients in remission, four patients in clearance, two
patients cured and five patients with recurrences. Comparing with the
international literature there were variations in the epidemiology due
to the type of population of the referral center.
Conclusions: The results obtained with the CO
2 laser were similar to
those reported in the international literature regarding fewer recurrences,
less bleeding, less surgical time and similar late complications, confirming
that resection must be limited to prevent stenosis. We suggest to
promote training of Mexican otolaryngologists to extend the use of this
surgical technique in patients with recurrent respiratory papillomatosis.
