Shoulder bipolar arthroplasty in acute fractures. A 30 case review

ABSTRACT
A retrospective study was performed on 30 patients who were implanted a partial shoulder prosthesis of the double-cupule type with a following time of 20 months. From the 30 patients 9 (30%) belonged to the male sex and the other 21 (70%) to the female sex. The right shoulder was treated in 23 cases (77%) and the left one in 7 cases (23%). The patients’ average age was 66.4 years. The type of fracture was of four fragments in 18 cases (60%), three fragments in 2 cases (7%) and fracture-luxation of two or more fragments in 6 cases (20%). The result evaluation was carried out through the Friedmann’s Shoulder Score System. The obtained average scoring was of 26.5 points, with a minimum of 17 and a maximum score of 28.4, ranging from excellent: 4 (13%); good: 18 (60%): average: 6 (20%); and bad: 2 (7%). We found no pain in 90% of the patients after one-year treatment. With regard to mobility, the average flexion was of 118° and the average abduction of 89°. From the x-ray study every case bituberous consolidation was verified, with the emergence of ectopic calcification in 8 patients (27%). With regard to complications, it stands out a case of luxation, 2 cases of hypertrophic cicatrization, 2 cases of clamping syndrome and a humerus fracture. The obtained results match the ones achieved in other published series, so we consider that the shoulder bipolar prosthesis is a good therapeutic method when treating humerus-head fractures and luxations.
