ABSTRACT
Microcirculation is a very important concept. Plastic surgery problems and their solutions depend mostly on the knowledge of microcirculation which is composed of vascular networks interconnected and regulated by several mechanisms. Blood pressure declines progressively as blood travels from the heart to the capillaries. As the flap is elevated, ischemia appears in different degrees, and so microcirculation changes toward a better perfusion. Nevertheless, ischemia can progress to flap necrosis as circulation is insufficient. Fluid movement through capillaries is controlled by hydrostatic and osmotic balances on the capillary membrane. There are several methods to assess flap viability: simple, clinical to complex.
