La microcirculación en la cirugía de colgajos

RESUMEN
La microcirculación es un concepto de gran trascendencia. En la cirugía plástica los problemas y soluciones dependen en gran medida del conocimiento de la microcirculación, misma que se compone de una fina red vascular interconectada y regulada por diversos mecanismos. La presión sanguínea disminuye progresivamente conforme viaja la sangre del corazón a los capilares. Al levantarse un colgajo se le somete a isquemia de diversa magnitud, así, se reordena la microcirculación con el fin de mejorar su perfusión. Sin embargo, esta isquemia puede llevar a la necrosis del colgajo si la circulación no se recupera o es insuficiente. El movimiento de líquido a través de los capilares está controlado por el balance de fuerzas hidrostáticas y osmóticas en la membrana capilar. Existen diversas maneras de monitorear un colgajo, desde clínicas sencillas hasta las más sofisticadas pruebas.
