Floating elbow associated to clavicle fractures and ipsilateral brachial plexus injury. A case report

ABSTRACT
Objective. To present a multiple injuries in the upper extremity, to analyze the mechanism of injury, evaluate the prognostic, treatment and review of the literature. Material and methods. We present a 19 years-old male who suffered a car accident with open humeral fracture and ipsilateral Ulna, Radius and clavicle fractures associated to brachial plexus injury treated by osteosynthesis. Results. The surgical treatment consist in closed intramedullary locking nail for the humerus, two DCP plates and 3.5 screws for the radius and ulna and reconstruction plate for the clavicle. The mechanism of injury was high-impact trauma at the same time; the prognosis for the brachial plexus lesion is not good. Conclusions. This is an uncommon injury and there are few reports in the literature, these are caused by high-impact trauma like in car accidents or falling from highs, in general the prognosis is bad because the multiple lesions associated to nerve injuries, however, the appropriate surgical treatment using the basic principles of osteosynthesis and early physical therapy.
