Pancreaticoduodenectomy: a 6 year-experience in a concentration hospital

ABSTRACT
Background: Pancreatic adenocarcinoma is the most common tumor in this gland. His prognosis remains bleak due mainly to late diagnosis, which means that only between 10 and 25% of these tumors are candidates for surgical treatment and the survival rate at five years is less than 10%.
Material and methods: We reviewed the clinical records of patients with biliopancreatic malignancy, who have been made a pancreaticoduodenectomy with or without pylorus preservation in the time period from 2006 to 2012.
Results: The age-related mortality of patients, we found that the average age of the patients who died was 62 years. Showing a larger trend of deaths in elderly patients (p = 0.6) and male. The histopathological result reported 90% adenocarcinoma of the head of the pancreas or Vater’s ampulla. The median survival was 34.3 months to date, with a lower limit of 29 months and up to date of 39.5 months.
Conclusions: No significant difference in terms of complications, survival and tumor recurrence between the different techniques used for pancreaticoduodenectomy. The histological type of the tumor and the size of it are determining factors in prognosis. There is a larger trend of mortality in elderly patients. With proper patient selection, adequate postoperative results can be obtained over time.
