Benign paroxysmal positional vertigo of horizontal semicircular canal (BPPV-HSC)

ABSTRACT
Benign paroxysmal positional vertigo is characterized by brief episodes of mild to severe episodes of vertigo, accompanied by nistagmus, which is
provoked by positional changes and can be reproduced by adopting the triggered position. The benign paroxysmal positional vertigo of horizontal
semicircular canal (BPPV-HSC) has an incidence ranging from 5 to 15%. It is the most difficult one for treatment. There are several maneuvers
which are very useful for the treatment of BPPV-HCS like the Barbecue maneuver, Gufoni, Vannucchi, Vannucchi-Asprella. We report the case of
a 74 year old man with a history of 6 months of recurrent episodes of disabling rotatory vertigo, lasting seconds, triggered with postural head
movements to the left, accompanied by nausea, sweating, vomiting, tachycardia and improved by visual afference. A hearing test and a
videonystagmography were requested. It detects a left horizontal nystagmus with the left roll-test diagnostic maneuver. Repositioning canalith
maneuvers were performed with consequent resolution of the clinical picture.
