Can and should ILAE classification be replaced by semiology-based classification?

ABSTRACT
Introduction: The currently accepted classification of the
International League Against Epilepsy is not a semiologic
classification of the epileptic seizures.
Objective: To determine
whether it is possible to obtain a semiologic
classification of the epileptic seizures based on history
taking and by this, to estimate location and laterality of
the epileptic zone.
Methods: A cross-sectional multicenter
double-blind study was performed on 189 patients
referred to surgical evaluation from 2007 to 2011. A
standardized-structure questionnaire and video-telemetry
was used to collect the main information.
Results: Auras,
focal seizures, aphasia, ictal psychosis, postictal
confusion, head version, hemicranea and peri-ictal
amnesia allowed identification of the seizures onset. The
time sequence in which typical temporal auras appeared,
focal frontal seizures followed by focal or generalized
hemiconvulsion, or with lower-limb automatisms without
oral automatisms associated with confusion, depression
and the existence of a latency period allowed clear
differentiation of epileptic zone.
Conclusion: Peri-ictal
symptoms evaluated by video-EEG can be obtained by
a detailed history taking, which associated with seizures
signs and symptoms and analyzed in strict temporal order
could allow estimation of the laterality and location of
the epileptic zone.
